APP was designed to assist individuals in achieving, maintaining and maximising their potential
in accordance with the principals of person centredness

The term “Person Centred” refers to activities that are based upon what is important to the individual
from their own perspective. The person centred planning approach adopted by APP supports the person to
realise their own potential by taking control of the planning process, and implementing a variety of strategies
that empowers them to identify and act upon what is important to them, and what they want from their life.
