Programme Information.

Our programme called “Making Connections” refers to the hopes, aspirations and support needs of the person and
the facilities, the resources and interventions provided / available to meet their needs. Each person is the focus of our
programme planning. The programme is designed to enhance their Quality of Life and for each person to live a more
meaningful life in their community

We discover and act upon what is important to each person and take the steps to make it happen through the development
of their personal support plans.

Person centred planning stimulates community hospitality and enlists community members in assisting the focus person
to define and to work towards a desirable future.

Family involvement in the person centred planning process is often important because in many cases life stories and
experiences are not documented or known to service providers. Families can be an integral part of the process in
supporting the person to achieve their goals.

A person centred planning meeting is a vehicle for providing everyone involved with clear expectations of what
family members and friends can and cannot contribute.

Person centred planning cannot be treated as a technique but should be a
commitment. APP is committed to delivering this type of service.
